If the teacher or his/her students (placing first in state or national competition) have received recognition for educational experiences the ACT Department wishes to recognize both at the appropriate Board meeting. All recognitions will take place in the Board Room located on the first floor of the School Support Center located at 215 Manatee Ave. West.
Please fill out this form and return it to the ACT Department within 4 weeks of receiving the recognition to enable us to complete the appropriate materials to be placed on the Board meeting agenda.
